Guards with rifles escort Dr. Kumar Gaurav as he makes the rounds at his hospital on the banks of the Ganges River. They are there to protect him from the relatives of patients, including those suffering from COVID-19. The bodyguards are ensuring the safety of the last doctor still standing as other doctors have contracted the disease.

One of the biggest challenges facing Dr. Gaurav is families of COVID-19 patients barging into wards to see their loved ones, often wearing just the flimsiest of masks for protection. He's now the top official at one of just four COVID-19 hospitals selected to serve 100 million people. Dr. Gaurav worries about his exposure to the patients, but feels compelled to keep working.
