Malaysia's transport officials have cited "human error" for a train collision that has left over 200 people injured. An empty light-rail train was on a test run in a tunnel in downtown Kuala Lumpur on Monday evening when it collided head-on with another light-rail train carrying 213 people. Malaysian transport authorities say 47 of the passengers are in critical condition.
