The crew operating the freight train that derailed in East Palestine, Ohio, didn't have much warning before dozens of cars went off the tracks, and there is no indication that they did anything wrong, federal investigators said on Thursday, as the National Transportation Safety Board released a preliminary report into the fiery wreck that prompted a toxic chemical release and an evacuation. However, the board chair Jennifer Homendy said the accident was "100 percent preventable".
