A preliminary report depicted confusion in the cockpit shortly before an Air India jetliner crashed, killing 260 people last month, after the plane's engine fuel control switches almost simultaneously flipped from the "run" to the "cutoff" positions, starving the engines of fuel. The report does not explain how the switches could have flipped to the cutoff position, nor does it indicate whether there was any emergency requiring an engine cutoff.
