A carriage on one of Lisbon's most iconic tourist attractions, the Gloria funicular railway, derailed and crashed on September 4. Authorities have confirmed that at least 16 people were killed and 22 more injured. It is not yet clear what caused the carriage to derail, but local media reports say a cable fault is suspected. Portugal's Prime Minister Luis Montenegro has called the funicular railway crash "one of the biggest human tragedies of our recent history."
