You're on an alien spaceship made not of metal but your own flesh. You are the pilot, but the controls have been locked from the outside. You realize you are trapped, and you realize why. It wants you alive only to feed from you. For years, it digests you. You can see everything, you can feel everything. You can scream internally, but no sound comes out. You know the ending from scene one. And you have to watch it play out. That's amyotrophic lateral sclerosis (ALS). The real nightmare that cameras don't show.

Cai Lei is the man who decided to fight the spaceship anyway. And he's building a lifeboat for others, funding teams of scientists around the world. "I hope any team crosses the finish line – I don't care if it's mine," he says. "When faced with hardship, or even hopelessness, no matter how many times you fail to accomplish what no one has done before, do not be afraid. Even if the odds are stacked against you, never surrender. You will light a flame of hope for yourself and for the world."

Cai Lei is not alone in that fight. Doctors, scientists, caregivers, fellow patients and their families have come together to form a resilient community built on shared knowledge and mutual support. Through their collective efforts, they are driving real progress, developing new treatments, refining assistive equipment and pioneering technologies that give patients more time and a better quality of life.
