The crew of the USS Fitzgerald navy destroyer is believed to be at fault in last month’s Tokyo Bay collision that killed seven sailors.
According to an initial report by the US defense department, the crew members were not paying attention to their surroundings at the time of the accident. But officials stressed that the investigation was still in its “early stages” and it was too soon to come to conclusions.
The US warship and a Philippine container ship collided in clear weather south of Tokyo Bay on June 17. It was the largest loss of life on a US navy vessel since 2000.
The captain of the Philippine cargo ship said the US warship had failed to respond to warning signals or avoid his vessel before the collision.
